Holiday Application and Personal Support Form

This is the form to complete detailing your interests and medical and social needs. Please complete this form and send to Leisure Options.  Leisure Options staff will access your application and determine the correct holiday support ratio you will require.

Travel Insurance Brochure and Application

This brochure details the travel insurance policy and conditions. There is a travel insurance application form on page 26. Print  page 10 as a PDF only. This will need to be printed and competed and returned to Leisure Options with payment. Please contact Leisure Options if you would like to receive a travel insurance application brochure by mail

If you would like Leisure Options to plan a specific holiday for you or a group of people. Please compete this form and return to Leisure Options with the planning fee

If you have a medical condition that is not covered in the "Self Assessment Condition (See your Cover More Insurance Brochure) you maybe able to be covered by applying using this form.
